Student Medical Statement

REfund Policy

1. All Instructional Programs and Services Are Non-Refundable:

  • Fees for our scuba diving programs and services are non-refundable. Once paid, enrollments cannot be refunded. It is recommended that, prior to enrolling in any program, you are medically fit or cleared for scuba diving activities. The student medical statement can be found below.

2. All Sales Are Final:

  • Please note that all sales, including programs, services, and related items, are final. No refunds will be issued.

3. Exchange for Store Credit (Unused gear items):

  • Unused items can be exchanged for store credit if they meet the following conditions:

    • Item is unused.

    • You have a valid receipt.

    • The item is in original packaging with all materials, manuals, and warranty cards.

    • Item is brought back within 20 days of purchase.

    • Return on items are subject to a restocking fee.

    • Certain items, like books, CDs, and DVDs, cannot be returned if opened due to copyright laws.

4. Transfer of Instructional Services/Programs (If Needed):

  • While instructional services/programs are non-refundable, we allow transfers:

    • Notify us in advance.

    • Provide details of the new participant.

    • Ensure they meet eligibility requirements.

    • Discuss any associated fees or changes (eLearning).

    • The original participant is responsible for coordination.

Reschedule Policy

  • Rescheduling Confined Water Sessions: minimum of 48hr notification to avoid fee's.

  • Rescheduling Open Water Sessions: minimum of 48 hr notification to avoid fee's.

Travel Bookings

  • DiVE KS offers travel bookings and services. Please note that all bookings done through DiVE KS are subject to a booking fee.

Student MEdical Statement 

A Student medical statement is required for all PADI scuba diving programs. When you enroll a copy of the student medical statement will be provided to you by email and on your PADI eLearning student dashboard. In certain instances, a medical provider may need to give permission or medical clearance to dive. Please feel free to look over the student medical statement and show it to your Primary Care Provider if needed. This must be completed prior to any in-water activity. There are no ecxeptions.
